The Pancake skillshot is achieved by smashing an enemy to death against the wall by kicking a barricade against him. This can be done three times during Act 1 Chapter 2: twice after chasing several Skulls who throw barricades down in front of them, and once again right before charging Ishi's battery. It can also be performed in Act 6 Chapter 2 when fighting the Burnouts.
